OVERVIEW OF OSTEOARTHRITIS
A derivation from the Greek words for inflammation (itis), joint (arthr) and bone (osteo), Osteoarthritis also known as Degenerative Joint Disease is a chronic, slowly progressing condition involving the joints that is caused by the breakdown of cartilage and other joint tissues usually due to bone deterioration. Although it can occur in any joint in the body it predominately affects the weight bearing joints such as the hips and the knees, and less frequently the ankles, shoulders, wrists and hands. It usually starts in one area and develops in other areas as the condition progresses.
INCIDENCE AND PREVALENCE
Current statistics show that approx 2% of Americans under the age of 45 suffer from Osteoarthritis. That number significantly rises with age, affecting 30% of those between the ages of 45 and 64 and 63-85% of those over 65 years of age. Though more men than women under the age of 45 have osteoarthritis, the number markedly changes as the person ages. After the age of 45 the condition is much more prevalent in women
CAUSES
• The aging process can be a factor in the degeneration of the joints. Wear and Tear of the joints can also be
caused by the prolonged wear and tear experienced by athletes.
• Joint injury or stress – Fractures and infection can damage the joints, this is seen in athletes who have
repeated injuries. Repetitive stress to a joint, common in certain occupations such as dancers, computer keyboard operator and assembly line operator.
• Heredity – Some research studies have found a correlation between certain genetic mutations and the increased
likelihood of developing osteoarthritis
• Muscle weakness – usually caused by inactivity. Joints that are underused often become stiff and dysfunctional
and are at high risk for development of osteoarthritis
• Obesity – The extra weight puts a strain on the joints especially those that are weigh bearing.
COMMON SYMPTOMS OF
OSTEOARTHRITIS INCLUDE:
• Pain - Usually present in the joints of the hands, hips, knees and feet and at times in the spine.
Initially the pain is associated with activity and therefore the pain becomes worse at the end of the day or after being active. However, as the condition progressively worsens, the patient experiences pain even during the rest periods.
• Stiffness - Usually of short duration following prolonged periods of inactivity such as night’s sleep or sitting for an extended period of time.
• Restricted joint motion
• Swelling of joints
• Tenderness at or around joints
• Grating sensation upon use of joints
• Bone Spurs – extra bone around joints.
CONVENTIONAL TREATMENT: Conventional treatment usually includes:
• Physical therapy: teaching exercises such as swimming, walking, and water aerobics and strengthening and stretching;
• Occupational therapy: Guidance in performing activities of daily living and utilizing assistive devices such as
canes, crutches, splints and braces to reduce the stress on the joints.
• Surgery: Surgery is considered as a late stage intervention and might include: replacement or reconstruction
of joints, realignment or fusing of bones.
Medications: • Initial phases medications: Non-steroidal anti-inflammatory drugs or
corticosteroids such as prednisone.
• Later phases Drug Therapy, as symptoms worsen: Injection of gold salts, anti-malarial drugs and immunosuppressive
drugs.
