OVERVIEW OF PAIN CAUSED BY MENSTRUAL CRAMPS
Pain caused by the contraction of the uterus as it expels blood and the uterine lining. The pain is spasmodic and cramping in nature and it may be dull or throbbing. . It typically occurs in the lower abdomen and may radiate into the groin, thighs, and lower back. Its onset is usually at the beginning of the menstrual flow when it is most severe. It may last a few hours or up to 1-3 days

INCIDENCE AND PREVALENCE
Menstrual cramps affect more than one third of all women. Approximately 50% of those suffering from menstrual cramps describe them as severe. It is one of the most common health problem women experience during their reproductive years. The incidence is more prevalent in the younger woman from teen years to their thirties. Many hours of work are lost due to this disorder as the monthly symptoms great affect the functionality of many women.

CAUSES FOR PRIMARY DYSMENORRHEA or MENSTRUAL CRAMPS
During a woman’s childbearing years, every month the inner lining of the uterus or endometrium regrows to prepare for a possible pregnancy. If the egg is not fertilized by the sperm after ovulation and therefore no resulting pregnancy, the lining is superfluous. The woman’s hormones progesterone and estrogen decrease and the body expels the unneeded uterine lining.

As the endometrium starts to break down hormone-like substances called Prostaglandins are released. These substances set off uterine muscle contractions which in turn cause vasoconstriction restricting blood flow to the endometrium. Diminished blood flow to the uterine lining limits the oxygen it receives, causing it to die. That dead tissue is then expelled from the body with continued muscle contraction during the menstrual cycle. The uterine contractions as well as the poorly oxygenated tissue are thought to cause the pain. It has been shown that women with high levels of prostaglandins have more severe menstrual cramps. Another reason for the increased intensity of the cramping may be due to clots and pieces of uterine tissue pass through the cervix.

Leukotrienes which are chemical compounds involved in the inflammatory process are increased during the menstrual period and may be another culprit in the development of painful cramps.

CAUSES FOR SECONDARY DYSMENORRHEA or MENSTRUAL CRAMPS
These causes are due to a disease process such as:
  • Endometriosis – A painful condition in which the uterine tissue becomes embedded in areas outside the uterus such as the fallopian tubes, ovaries or pelvis.
  • Uterine Fibroids – Though rare, these growths or tumors in the wall of the uterus cause pain.
  • Pelvic Inflammatory Disease – Often caused by sexually transmitted pathogens, this painful condition involves infection of the female reproductive organs.
  • Adenomyosis – The tissue that covers the uterus expands into the muscular layer of the uterus. The main symptoms of this condition are uterine pain and bleeding.
  • Cervical Stenosis: A narrow cervical opening inhibits menstrual flow thereby causing an elevated painful pressure within the uterus.

COMMON SYMPTOMS OF MENSTRUAL CRAMPS INCLUDE:
  • Pain and cramping beginning in the lower abdomen or pelvis
  • Pain can radiate to lower back and legs
  • Pain can be continual or periodic
  • Cramping can accompanied by:
  • Headaches
  • Nausea
  • Vomiting
  • Diarrhea or constipation
  • Urinary urgency
  • Bloating
  • Dizziness
  • Malaise

CONVENTIONAL TREATMENT:
Conventional treatment for primary menstrual cramps includes prescription non steroidal anti inflammatory drugs, over the counter non steroidal anti inflammatory drugs, and for mild cramps aspirin or Tylenol. Some doctors utilize hormonal birth control methods in order to prevent ovulation and decrease the intensity of the cramps.

Surgery may be an option for those women suffering from secondary menstrual cramps. The surgical excision of the offending tissue may aid in the reduction or elimination of the pain.

ALTERNATIVE TREATMENT:
